What is a Case Manager?
A registered case manager is a qualified health or social care professional who coordinates care across multiple services, stakeholders, and settings on behalf of an individual with complex needs.
Unlike a single clinician who focuses on one aspect of a person's health, a case manager holds the whole picture — ensuring that medical, psychological, social, occupational, and legal strands of a case are aligned and progressing.
In the UK, case managers are registered through professional bodies including the Institute of Registered Case Managers (IRCM) and CMSUK, ensuring adherence to national standards of practice.
Our Social Work Case Manager, Bridgene Doyle, is a qualified Social Worker with extensive experience in complex care coordination, currently registered with NISCC and Case Management Society UK (CMSUK).

This tier provides structured oversight and guidance without intensive involvement. It is ideal where individuals need help navigating systems, clarifying next steps, or coordinating existing supports.

Tier 2: Active Case Management
For moderate complexity or stalled cases.
This level offers more regular involvement and active coordination, supporting individuals where progress has slowed or multiple stakeholders are involved.
